Serve a pivotal role bringing individualized care to the client's home as an alternative to hospitalization.
Reports to: Clinical Director/RN
Provide direct nursing care under the supervision of an RN, helping reduce hospital admissions and ER utilization.
Reports to: Clinical Director/RN or Operations Director
Provide direct medical and non-medical care to clients in the home, a level above Home Health Specialist I.
